Each permittee shall submit a calendar-year annual report to the division by no later than March 31 of the next year.\n\nB. The annual report shall include as appropriate:\n\n1. A confirmation of the accuracy of the permittee's current registration filed with the division or a report of any change in the information;\n\n2. The name, address and phone number or numbers of the persons to be contacted at any time in case of an emergency;\n\n3. Production of gas or oil on a well-by-well and county-by-county or city-by-city basis for each permit or as prescribed by the director and the average price received for each Mcf of gas and barrel of oil;\n\n4. Certification by the permittee that the permittee has paid all severance taxes for each permit;\n\n5. When required, payment to the Gas and Oil Plugging and Restoration Fund as required in § 45.2-1634 of the Code of Virginia; and\n\n6.
