Any petition to the board not otherwise provided for in this chapter shall be made in writing, and shall contain:\n\n1. The name and address of the petitioner and the petitioner's counsel, if any;\n\n2. The names and addresses of any persons who are named as respondents in the petition;\n\n3. An affidavit that notice has been given to each respondent, if any, named in the petition;\n\n4. A statement of the issues of the petition; and\n\n5. A statement setting forth the specific relief requested.\n\nB. If a petitioner for a unit under § 45.2-1616 or § 45.2-1622 fails to provide notification to an owner of interest of any part of a unit subject to a petition before the board, then such party may file a written objection to the proceedings in the form of a petition as set out in subsection A of this section.
