A form for the submission is available from the department. The information shall include the following:\n\n1. Name and location information for the impounding structure, including city or county and latitude and longitude;\n\n2. Name of owner and operator and associated contact information, including residential and business telephone numbers and other means of communication;\n\n3. Contact information for the local emergency management coordinator;\n\n4. Procedures for notifying downstream property owners or occupants potentially impacted by the impounding structure's failure;\n\n5. Identification of any downstream roadways that would be impacted by the impounding structure's failure; and\n\n6. Certification by the owner that the plan is accurate and that the owner understands the responsibilities included in the plan.","path":["Title 4. Conservation And Natural Resources","Agency 50.
