The facility shall implement written procedures governing case management services, which shall address:\n\n1. The resident's adjustment to the facility, group living, and separation from the resident's family;\n\n2. Supportive counseling, as needed;\n\n3. Transition and community reintegration planning and preparation; and\n\n4. Communicating with (i) staff at the facility; (ii) the parents or legal guardians, as appropriate and applicable; (iii) the court service unit; and (iv) community resources, as needed.\n\nB.
