Separate up-to-date personnel records shall be maintained for each full-time and part-time employee, student intern, and volunteer for whom background investigations are required by Virginia statute. Content of personnel records of volunteers, student interns, and contractual service providers shall include, at a minimum, documentation of compliance with requirements of Virginia laws regarding child protective services and criminal history background investigations.\n\nB.  Records shall be maintained for each staff to include:\n\n1. A completed employment application or other documentation providing the individual's name, address, and telephone number;\n\n2. Documentation of qualifications, including educational background and professional licensure or certification;\n\n3. Employment history;\n\n4. Written references or notations of oral references;\n\n5. Reports of required  tuberculosis certificate;\n\n6. Annual performance evaluations;\n\n7. Date of employment for each position held and date of separation;\n\n8. Documentation of compliance with requirements of Virginia laws regarding child protective services and criminal history background investigations;\n\n9.  Driving record verification from the Department of Motor Vehicles  and a current copy of the driver's license for  staff who transport students;\n\n10. Documentation of all training required by this chapter and any other training or professional development received by individual staff; and\n\n11. A current job description.\n\nC. All personnel records shall be maintained confidentially and retained in their entirety for a minimum of three years after staff's separation from the school.","path":["Title 8.
