Name, address, verification of age requirement, job title, and date of employment or volunteering; and name, address, and telephone number of a person to be notified in an emergency which shall be kept at the center.\n\n2. For staff hired after March 1, 1996, documentation that two or more references as to character and reputation as well as competency were checked before employment or volunteering. If a reference check is taken over the phone, documentation shall include:\n\na. Dates of contact;\n\nb. Names of persons contacted;\n\nc. The firms contacted;\n\nd. Results; and\n\ne. Signature of person making call.\n\n3. Background checks as required by the regulation entitled Background Checks for Licensed Child Day Programs and Family Day Systems (8VAC20-770).\n\n4. Documentation to demonstrate that the individual possesses the education, certification, and experience required by the job position, and orientation and training as required in 8VAC20-780-240 and 8VAC20-780-245.\n\n5. First aid, cardiopulmonary resuscitation, and other certifications as required by the responsibilities held by the staff member.\n\n6. Health information as required by 8VAC20-780-160 and 8VAC20-780-170.\n\n7. Information, to be kept at the center, about any health problems that may interfere with fulfilling the job responsibilities.\n\n8. Date of separation from employment.","path":["Title 8.
