The provider shall record in the child's record an injury or accident sustained by a child while at the family day home that requires first aid or emergency medical or dental treatment.\n\nB. The information recorded shall include the following:\n\n1. Date and time of injury;\n\n2. Name of injured child;\n\n3. Type and circumstance of the injury;\n\n4. Caregiver present and action taken;\n\n5. Date and time when parents were notified;\n\n6. Any future action to prevent recurrence of the injury;\n\n7.
