The provider shall report to the department within 24 hours of the circumstances surrounding the following incidents:\n\n1. Lost or missing child when local authorities have been contacted for help;\n\n2. Serious injury to a child while under the family day home's supervision; and\n\n3. Death of a child while under the family day home's supervision.\n\nB.
