The department will make application forms available to applicants. The information required by this section shall be determined and submitted according to procedures and methods acceptable to the department.\n\nB. Each initial application for coverage under the general permit shall include, but not be limited to, the following:\n\n1. Information specified in the appropriate air permit application form for an affected unit as determined by the department.\n\n2.
