Except as provided in § 38.2-4912, no provider shall engage in the business of providing or offering to provide continuing care at a facility in this Commonwealth unless the provider has registered with the Commission with respect to such facility.B. A registration statement shall be filed with the Commission by the provider on forms prescribed by the Commission and shall include:1. All information required by the Commission pursuant to its enforcement of this chapter; and2. The initial disclosure statement required by § 38.2-4902.C.
