{"data":{"id":"us-vt/18-v.s.a.-704","jurisdiction":"us-vt","citation":"18 V.S.A. § 704","heading":"Medical home","body":"(a) Consistent with federal law to ensure federal financial participation, a health care professional providing a patient’s medical home shall:\n(1) provide comprehensive prevention and disease screening for his or her patients and managing his or her patients’ chronic conditions by coordinating care;\n(2) enable patients to have access to personal health information through a secure medium, such as through the Internet, consistent with federal health information technology standards;\n(3) use a uniform assessment tool provided by the Blueprint in assessing a patient’s health;\n(4) collaborate with the community health teams, including by developing and implementing a comprehensive plan for participating patients;\n(5) ensure access to a patient’s medical records by the community health team members in a manner compliant with the Health Insurance Portability and Accountability Act; 12 V.S.A. § 1612; sections 1852, 7103, 9332, and 9351 of this title; and 21 V.S.A. § 516; and\n(6) meet regularly with the community health team to ensure integration of a participating patient’s care.\n(b) A naturopathic physician licensed pursuant to 26 V.S.A. chapter 81 may serve as a patient’s medical home.","path":["Title 18: Health","Chapter 013: Chronic Care Infrastructure and Prevention Measures","Subchapter 001: BLUEPRINT FOR HEALTH"],"source_url":"https://legislature.vermont.gov/statutes/section/18/013/00704","current_through":"2025 session","vintage":"","retrieved_at":"2026-09-05T17:17:01Z","sha256":"2d7ce4399e6bb16e50f97eef3fec1d38f23266b6d5601c1bddeb42e3dd8ace88","source_id":"us-vt","stale":false,"prev":"us-vt/18-v.s.a.-703","next":"us-vt/18-v.s.a.-705"},"notice":"GroundRules: Original legal text.
