Such reports must contain the following information, if known:\n(1) The name, address, and age of the child;\n(2) The name and address of the child's parents, stepparents, guardians, or other persons having custody of the child;\n(3) The nature and extent of the alleged injury or injuries;\n(4) The nature and extent of the alleged neglect;\n(5) The nature and extent of the alleged sexual abuse;\n(6) Any evidence of previous injuries, including their nature and extent; and\n(7) Any other information that may be helpful in establishing the cause of the child's death, injury, or injuries and the identity of the alleged perpetrator or perpetrators.","path":["Title 26","Chapter 26.44"],"source_url":"https://app.leg.wa.gov/rcw/default.aspx?cite=26.44.040","current_through":"July 15, 2026","vintage":"","retrieved_at":"2026-09-06T01:03:42Z","sha256":"88ae60c84610e64463326b4d48c04fdaddb72d7fda6cb6c774acb2b2dc13c7c9","source_id":"us-wa","stale":false,"prev":"us-wa/rcw-26.44.035","next":"us-wa/rcw-26.44.050"},"notice":"GroundRules: Original legal text.
