{"data":{"id":"us-wa/rcw-69.43.040","jurisdiction":"us-wa","citation":"RCW 69.43.040","heading":"Reporting form.","body":"(1) The department of health, in accordance with rules developed by the pharmacy quality assurance commission shall provide a common reporting form for the substances in RCW 69.43.010 that contains at least the following information:\n(a) Name of the substance;\n(b) Quantity of the substance sold, transferred, or furnished;\n(c) The date the substance was sold, transferred, or furnished;\n(d) The name and address of the person buying or receiving the substance; and\n(e) The name and address of the manufacturer, wholesaler, retailer, or other person selling, transferring, or furnishing the substance.\n(2) Monthly reports authorized under RCW 69.43.010(4) may be computer-generated in accordance with rules adopted by the department.","path":["Title 69","Chapter 69.43"],"source_url":"https://app.leg.wa.gov/rcw/default.aspx?cite=69.43.040","current_through":"July 15, 2026","vintage":"","retrieved_at":"2026-09-06T03:47:18Z","sha256":"a2a52c6d6e0d6f22da2290932f24679a6f503b2df44ae2053972e0b7643bbafb","source_id":"us-wa","stale":false,"prev":"us-wa/rcw-69.43.035","next":"us-wa/rcw-69.43.043"},"notice":"GroundRules: Original legal text.
