The action plan for the consortium must include a Standard Form 424 for the consortium for the HOME program. Each entitlement jurisdiction also must submit a Standard Form 424 for its funding under the CDBG program and, if applicable, the ESG and HOPWA programs.\n(b) Description of resources and activities. The action plan must describe the resources to be used and activities to be undertaken to pursue its strategic plan.
