{"data":{"id":"us/25-cfr-115.407","jurisdiction":"us","citation":"25 CFR 115.407","heading":"How is an address of record for a minor's supervised account changed?","body":"(a) To change an address of record for a minor's supervised IIM account, a custodial parent(s), legal guardian, or emancipated minor must provide BIA with the following information:\n(1) The minor's or emancipated minor's name;\n(2) The name of the custodial parent(s) or legal guardian, if applicable;\n(3) A custody order from a court of competent jurisdiction or a copy of a guardianship, if applicable;\n(4) The new address of the custodial parent(s), legal guardian, or emancipated minor; and\n(5) The signature, mark or thumb print of a custodial parent, legal guardian, or emancipated minor that has been notarized by a notary public and/or witnessed by a DOI employee who has been shown verifiable photo identification.
