{"data":{"id":"us/40-cfr-1604.4","jurisdiction":"us","citation":"40 CFR 1604.4","heading":"Information required in an accidental release report.","body":"The report required under § 1604.3(c) must include the following information regarding an accidental release as applicable:\n(a) The name of, and contact information for, the owner/operator;\n(b) The name of, and contact information for, the person making the report;\n(c) The location information and facility identifier;\n(d) The approximate time of the accidental release;\n(e) A brief description of the accidental release;\n(f) An indication whether one or more of the following has occurred:\n(1) Fire;\n(2) Explosion;\n(3) Death;\n(4) Serious injury; or\n(5) Property damage;\n(g) The name of the material(s) involved in the accidental release, the Chemical Abstract Service (CAS) number(s), or other appropriate identifiers;\n(h) If known, the amount of the release;\n(i) If known, the number of fatalities;\n(j) If known, the number of serious injuries;\n(k) Estimated property damage at or outside the stationary source; and\n(l) Whether the accidental release has resulted in an evacuation order impacting members of the general public and others, and, if known:\n(1) The number of persons evacuated;\n(2) Approximate radius of the evacuation zone; and\n(3) The type of person subject to the evacuation order (i.e., employees, members of the general public, or both).","path":["Title 40—Protection of Environment","CHAPTER VI—CHEMICAL SAFETY AND HAZARD INVESTIGATION BOARD","PART 1604—REPORTING OF ACCIDENTAL RELEASES"],"source_url":"https://www.ecfr.gov/api/versioner/v1/full/2026-08-25/title-40.xml","current_through":"2026-08-25","vintage":"","retrieved_at":"2026-08-27T02:26:04Z","sha256":"6ef9c454b2015acd18ae5a412fd38daef75969b89cd03ee85323ced97d13c879","source_id":"us-cfr","stale":true,"prev":"us/40-cfr-1604.3","next":"us/40-cfr-1604.5"},"notice":"GroundRules: Original legal text.
