Ark. Code Ann. § 14-120-212: Ballots

(a) The ballots for the election in the respective counties shall be prepared and distributed to the election officials by, or under the supervision of, the secretary of the district. (b) The ballot to be used at the election need not be in duplicate and shall be in substantially the following form: Click here to view form.
