Ark. Code Ann. § 5-5-404: Receipts

(a) (1) When any weapon or ammunition is transferred and delivered to the State Crime Laboratory, the laboratory shall provide a receipt to be signed by the transferor or donor and the laboratory officer or employee accepting the weapon or ammunition. (2) The receipt shall contain the following information: (A) A list of any weapon by type, make, and caliber; (B) The serial number of a weapon, when available; (C) The case number of the case in which the weapon was involved, when available; and (D) The type, caliber, and make of the ammunition, when available. (b) A copy of the receipt shall be retained by the laboratory and a copy of the receipt shall be delivered to the agency, individual, or other entity transferring or donating a weapon or ammunition.
