A.R.S. § 32-1389: Application for change of name

The owner of a licensed funeral establishment shall submit an application on a form prescribed by the department to change the name of the funeral establishment. The application shall be subscribed under oath and shall be accompanied by the applicable fee pursuant to section 32-1309.
