C.R.S. § 42-4-1608: Crash report forms.

(1) The department shall prepare and upon request supply to police departments, coroners, sheriffs, and other suitable agencies or individuals forms for crash reports required under this article 4, which reports must call for sufficiently detailed information to disclose, with reference to a traffic crash, the contributing circumstances, the conditions then existing, and the persons and vehicles involved.

(2) Every required crash report must be made on a form approved by the department, where such form is available.
