A notary shall use a jurat certificate in substantially the following form in notarizing a signature on an affidavit, deposition, or other sworn or affirmed written declaration:
“Territory of Guam.
Subscribed and sworn to before me this _____ day
of ________, 19____ by ____________________.
________________________
(official signature and seal of notary)”
