Haw. Rev. Stat. § 431:10-244: Filing procedure for contracts approved by commissioner.

Each insurance contract requiring approval by the commissioner pursuant to this code, section 392-48, or section 386-124 and each contract certified by the insurer to be in conformity with this code shall be accompanied by a $20 fee payable to the commissioner, which shall be deposited into the commissioner's education and training fund.
