Haw. Rev. Stat. § 443B-4.63: Furnishing deceptive forms.

It is unlawful for any person who is not a registered collection agency to design, compile, or furnish any form knowing that the form could be used to create a belief in a debtor that the person who designed, compiled or furnished the form is a collection agency.
