225 ILCS 217/45: Applications.

Each application for a license to practice under this Act shall be signed by the applicant on forms provided by the State Fire Marshal. Each application for a fire equipment distributor license shall be signed by an authorized officer of the fire equipment distributor. Each application for an employee license shall be signed by an authorized officer of the fire equipment distributor and by the candidate for the employee license.
