225 ILCS 705/3.01

The certificates provided for in this Act shall be issued under the signature of the Director and the seal of the Mining Board to all those who receive a rating above the minimum fixed by the rules of the Mining Board, and to no other persons. All certificates shall contain the full name of the recipient, his age, his place of birth, and the nature and number of years of his previous service in or about coal mines.
