430 ILCS 85/2-15.1

When contracting with any person or entity for the provision of an amusement ride or amusement attraction, the amusement ride or amusement attraction owner or operator shall inform the Department of the name and address of the operator, as well as the dates on which the amusement ride or amusement attraction will be operated pursuant to the contract, the person or entity contracting with the owner or operator, and the location at which such ride or attraction will be so operated.
