625 ILCS 5/1-159.2: Personally identifying information.

Information that identifies an individual, including his or her driver's license number, name, address (but not the 5 digit zip code), date of birth, height, weight, hair color, eye color, email address, and telephone number, but "personally identifying information" does not include information on vehicular crashes, driving violations, and driver's status.
