IC 22-15-5-4.5: Requirements to submit documentation

Sec. 4.5. The department may require submission of documentation, including payroll records or records of hours worked by employees, that demonstrates that work performed on a regulated lifting device under sections 1 and 4 of this chapter was completed by a qualified person as described under this chapter.
