IC 6-8.1-8-11: Filing claim for reimbursement

Sec. 11. A claimant for reimbursement under section 10 of this chapter must file a claim:

(1) with the department;

(2) on a form prescribed by the department; and

(3) not later than ninety (90) days after the date of the levy.
