KRS 334A.130: License application -- Form -- Fee.

(1) Each person desiring to obtain a license from the board shall make application to

the board. The application shall be made upon a form and in such a manner as the

board prescribes in regulations duly adopted under this chapter.

(2) The application shall be accompanied by the application fee as determined by the

board and set forth by regulations. This fee shall not be refunded by the board.
