KRS 342.430: Blank forms to be furnished by commissioner.

The commissioner shall prepare and furnish, free of charge, blank forms of all notices,

claims, reports, proofs, and other blank forms and literature which he or she considers

proper and requisite to the efficient administration of this chapter. He or she may

authorize the publication and distribution of these blanks by employers and their insurers

in manner and form provided by him or her, and shall promulgate administrative

regulations for their distribution so that they may be readily available.
