Mass. Gen. Laws ch. 32A, § 7: Information furnished commission by employees

Section 7. Each employee to whom this chapter applies shall furnish the commission, in such form as it shall prescribe, such information as is necessary to enroll himself, or himself and his dependents, under the policy or policies of insurance.
