Sec. 1131.

If a license or registration issued under this article is lost or destroyed, the department shall issue a new license or registration, without examination, if a fee of $30.00 is paid and an application for a new license or registration is submitted, accompanied by a written statement made by the licensee or registrant that the license or registration was lost or destroyed.
