Sec. 5.

The terms of the appropriation authorized by this act shall specify:

Name, address, and general purpose of the organization.

A description of the functions and responsibilities to be performed by the recipient of the appropriation.

The effective date and length of the grant.

Program and financial reporting requirements as established by the local unit of government.
