Minn. Stat. § 176.271: INITIATION OF PROCEEDINGS.

Subdivision 1. Written petition.

Unless otherwise provided by this chapter or by the commissioner, all proceedings under this chapter are initiated by the filing of a written petition on a prescribed form with the department or office. All claim petitions shall include the information required by section 176.291.
