Mo. Rev. Stat. § 287.630: Forms, other material furnished by commission and division.

The division and the commission shall prepare and furnish free of charge blank forms of all notices, claims, reports, proofs, and other blank forms and literature which they may deem proper and requisite to the efficient administration of this chapter. They may also authorize the publication and distribution of the blanks by employers and other persons.
