Mo. Rev. Stat. § 294.070: Work certificate forms furnished by state commissioner of education — no fee.

No fee shall be charged for a work certificate or work permit issued under the provisions of this chapter. The form of the work certificate shall be prepared by the state commissioner of education. Blank work certificates shall be furnished to the superintendent of each school district by the state commissioner of education.
