Mo. Rev. Stat. § 78.100: Candidate to file statement.

Any person desiring to become a candidate for mayor or councilman shall file with the city clerk a statement of such candidacy in substantially the following form:

State of Missouri, County of ______ ss.

I, ______, herewith state and declare that I reside at number ______ street, city of ______, county of therein; that I am a candidate for nomination to the office of (mayor or councilman) to be voted upon at the primary election to be held on the ______ Tuesday of ______, 20______, and I hereby request that my name be printed upon the official primary ballot for nomination by such primary election for such office.
