RSA 266-A:9: Applications for Certificates and Permits.

Applications for certificates and permits required by RSA 266-A:4 and RSA 266-A:7 shall be:

I. Made in writing to the department;

II. Verified under oath;

III. Accompanied by the proper fee;

IV. In the form specified in rules adopted by the commissioner pursuant to RSA 541-A; and

V. With the information specified in rules adopted by the commissioner pursuant to RSA 541-A.
