NRS 616C.005: Forms for reporting injuries: Insurer to distribute revised forms to employers periodically; Administrator to make revised forms available to physicians, chiropractic physicians, physician assistants and advanced practice registered nurses.

On or before September 1 of each year:

1. An insurer shall distribute to each employer that it insures any form for reporting injuries that has been revised within the previous 12 months.

2. The Administrator shall make available to physicians, chiropractic physicians, physician assistants and advanced practice registered nurses any form for reporting injuries that has been revised within the previous 12 months.
