Ohio Rev. Code § 4509.07: Contents of accident report.

The report prescribed by the registrar of motor vehicles shall request only information sufficient to enable the registrar to administer and enforce the provisions of sections 4509.01 to 4509.78, inclusive, of the Revised Code.

The person involved in an accident who submits or is the subject of a report submitted in accordance with section 4509.06 of the Revised Code shall furnish such additional relevant information as the registrar requires.
