Ohio Rev. Code § 4760.05: Issuing duplicate license.

On application by the holder of a license to practice as an anesthesiologist assistant, the state medical board shall issue a duplicate license to replace one that is missing or damaged, to reflect a name change, or for any other reasonable cause. The fee for a duplicate license is thirty-five dollars.
