Okla. Stat. tit. 56, § 56-166: Application for assistance

Application for assistance under this act shall be made to the

county department of the county in which the applicant resides. The

application shall be in writing, signed by the applicant or legal

guardian and shall be in a manner and upon the form prescribed by

the State Department.
