Okla. Stat. tit. 59, § 59-1210: Record of proceedings - Register of applications

The Oklahoma Department of Agriculture, Food, and Forestry shall

keep a register of all applications for registration. The register

shall show the name, age, and residence of each applicant; the date

of the application; address for the receipt of mail and the place of

business of each applicant; the education and other qualifications

of the applicant; whether or not an examination was required;

whether the application was rejected; whether a license was granted;

the date of the action of the Department; and such other information

as may be deemed necessary by the Department.
