Okla. Stat. tit. 59, § 59-1916.1: Statement of Professional Disclosure - Copy to be

furnished to client.

All licensed professional counselors, except those employed by

federal, state, or local governmental agencies, shall, prior to the

performance of service, furnish the client with a copy of the

Statement of Professional Disclosure as promulgated by rule of the

State Board of Behavioral Health Licensure. A current copy shall be

on file with the Board at all times.
