Okla. Stat. tit. 59, § 59-497: Duplicate licenses

The State Board of Medical Licensure and Supervision is hereby

authorized to issue a duplicate license to any licensee of this

state, who may have lost his license except through suspension,

failure to renew, revocation or denial; provided, that the

application, properly verified by oath, be made upon forms provided

for that purpose; and provided, further, that a fee set by the Board

shall be paid.
