The coroner, before he returns such inquisition and evidence, shall endorse them in this form:

Inquisition taken this ______ day of __________, A. D. __________, by __________, coroner for said county, entered and recorded in Coroner's Book of Inquisitions, page ____, this ____ day of __________, A. D. __________."
