Tenn. Code Ann. § 62-4-115: Fee for replacement or correction of license

A fee as set by the board will be charged for:

(1) Replacement of any lost, misplaced or mutilated license; or

(2) Change of name or mailing address by any cosmetologist, aesthetician, manicurist, natural hair stylist or instructor licensed under this part.
