Tenn. Code Ann. § 62-5-304: Application for funeral establishment license

(a) Every person, partnership, firm, association or corporation desiring to commence the operation of a funeral establishment after January 1, 2008, shall make application to the board, along with payment of a nonrefundable fee as set by the board. (b) All applications shall show the: (1) Name of establishment; (2) Address of establishment; (3) Owner or owners of establishment; (4) Name and address of the licensed funeral director who will act as manager of the funeral establishment; (5) Names and addresses of all licensed funeral directors and embalmers, designating whether they are part-time or full-time employees; and (6) Names of all unlicensed assistants. (c) Each establishment location shall be licensed separately from any other funeral establishment. Acts 1968, ch. 542, § 3; T.C.A., § 62-511; Acts 1989, ch. 523, § 149; 2007, ch. 297, § 1.
