Tex. Insurance Code § 2054.301: APPLICATION FOR COVERAGE.

An application to the company for workers' compensation insurance coverage must be:

(1) made on the form prescribed by the company; and

(2) submitted directly by the applicant or by a general property and casualty agent on behalf of the applicant.
